This Page provides links to DSM-5 LEVEL ONE AND TWO – DISORDER ASSESSMENTS. LEVEL ONE assessments are the screening tools. These assessments provide screening data that assists in determining appropriate focus.
The original DSM-5 Scales are paper documents designed for client – written – data entry. These instruments are completed by the Client and then scored according to DSM guidelines. We have converted these documents to PDF Forms that accept client entries and automatically score the results in accordance with DSM guidelines.
These assessment and disorder results should be reviewed with a professional clinician so that appropriate treatment plans can be developed. iMResponsible.com has a repository of treatment modules that provide tele-therapy options. These should be reviewed with your Clinician and other treatment TEAM members.
